Two types are chronic bronchitis that results in increased airway resistance which may in- clude reversible or irreversible air- way narrowing and emphysema which causes decreased fimctional lung capacity due to loss of elas- ticity of the alveolL These condi- tions may be due to smoking, some occupational lung diseases or may be hereditary. COPD is a major cause of disability death, and mortality that continues to increase due to our aging population. In the United States, it is second only to heart disease as a cause of disability. While emphysema is more com- mon in men than women, the in- cidence in women is increasing, possible due to more women smok- COPD is thought to begin early in adult life, although sig- nificant symptoms and disability usually do not occur until middle age. Mild breathing difficulties may be noticed long before the onset of clinical symptoms. A mild "smokers cough" is often present many years before onset of exer- tional dyspnea or difficulty breath- mR. Gradually, progressive exer- tional dyspnea occurs, sometimes with cough, wheezing, recurrent respiratory infections and oc- casionally weakness, weight loss, or lack of libido. Late in the disease, there may be frank cyanosis, a bluish dis- coloration of the skin from lack of oxygen, or a red torrid complexion associated with increased blood cell production. Mild chronic de- pendent edema or swelling is com- mon but does not necessarily indi- cate heart failure. It may result from prolonged sitting, elevated in- trathoracic pressure and renal retention of salt secondary to blood gas abnormalities. Treatment does not cure, but relives symptoms and controls potentially fatal exacerbations. It also may slow progression of the disease. Therapy is aimed at al- leviating conditions that cause symptoms and excessive disab'dity. Avoidance of bronchial irritants, ...... especially smoking, is of prime ira-, port.ance.
